Note:
If you have medical insurance coverage, please include
a copy of your insurance card. A copy of both the front
and the back of your insurance card(s) is needed. There
must be a copy on file with your athletic participation packet
in order to start practice with your team. Failure to turn
in a copy of your insurance card(s) forfeits practice and
competition with your team.

First-time Garrett College Athletes who plan to receive scholarship
benefits must review and/or sign the following prior to the
start of school.
